Title page of the printed text of the play gives these details. It is to be supposed that the play was relatively popular and revived regularly as part of the King's Men's repertory, since it was included in a list of plays for which they sought protection in a Lord Chamberlain's order in 1640, preventing the listed works from being published without their permission. (See Bawcutt, p.268.)
